Clinical Services
If you are looking for a family doctor or nurse practitioner you can apply to become a clinical client. Please note that demand for clinical services is high, and there is currently a wait list. As a Community Health Centre, we prioritize people who experience barriers to accessing health care. Learn more about our priority populations on our (Who We Serve) page
How to Apply
To apply for clinical services:
Complete the Application for Clinical Services form.
Submit your completed application to the reception desk at East End Community Health Centre or via email to eastendreception@eastendchc.on.ca.
